What Are Annual Gynecological Exams

Annual gynecological exams are preventive check-ups that help monitor your reproductive health, screen for concerns early, and support you through each stage of your cycle and life.

Here’s why these yearly visits matter.

  • Early detection of conditions that may not yet cause symptoms.
  • Preventive care to stay ahead of potential health issues.
  • Reliable health education in a comfortable, supportive space.
  • Timely management and follow-up if any abnormalities or changes are found.

When to Begin an Annual Exam

You should begin annual gynecological exams by age 21, even if you’re not sexually active.

Once you are sexually active, regular gynecology visits are essential because sexual activity can increase the risk of infections, hormonal changes, and symptoms that need timely evaluation. Annual screenings and assessment of concerns can help protect your reproductive health.

If you’re experiencing irregular periods, pelvic pain, abnormal bleeding, or any new changes that don’t feel normal for your body, you may need to schedule a visit sooner.

What to Expect During an Annual Exam

Your annual exam is designed to be calm, comfortable, and centered around your needs. Our all-female team uses a trauma-informed approach because many people can feel vulnerable during pelvic and breast exams.

Our providers explain each step and ensure you're comfortable throughout your consultation, so you always feel safe, respected, and in control of the pace of your exam.

Here’s what your visit may include:

Check-in and review of your medical history to understand your current health

Breast and pelvic exams, performed gently and explained step by step

Pap smear and STI testing, when needed or requested by you

Discussions about birth control, hormones, or cycle changes, based on your needs and goals

A follow-up visit in case of urgent gynecological concerns

What’s Included in an Annual Exam

At About Women By Women, your annual exam is more than a routine check-up; it’s a dedicated time to focus on your health, comfort, and questions.

If English isn’t your first language, several of our providers also speak Spanish, Russian, Greek, or Japanese, so you can share concerns in the language that feels most natural to you.

Here’s what your annual exam typically includes.

Breast Exam

A breast exam is a quick, hands-on assessment of your breast tissue to check for lumps, changes, or early signs of concern.

During this exam, our provider gently examines your breast for any unusual lumps or changes in skin texture. It takes only a few minutes and is done with care to keep you comfortable.

We also walk you through how to perform breast self-exams at home, giving you the confidence to notice changes early and stay vigilant about your own breast health between visits.

Pelvic Exam

A pelvic exam is a visual and hands-on inspection of your reproductive organs for signs of infection, irregular bleeding, or other gynecologic concerns. We talk you through each step so you feel safe, supported, and informed throughout the process.

Health Assessment

We also conduct a comprehensive health assessment that includes simple measurements, such as height, weight, and blood pressure, to provide a clear picture of your overall well-being. Our provider will also discuss your menstrual cycle, sexual activity, and any changes you may have noticed recently.

This conversation is relaxed and open-ended, so you can share what feels comfortable to you. Our goal is to understand what your body may need right now and how we can support your long-term wellness.

Pap Smear

A Pap smear is a simple screening test that checks the cells of your cervix for early changes that could lead to cervical cancer. During the exam, our provider gently collects a small sample of cells from your cervix using a soft brush or swab. The process takes only a few seconds and is painless.

The sample is then analyzed in a lab for any abnormalities so we can address concerns in a timely manner.

Routine Pap testing is recommended starting at age 21, even if you’re not sexually active. Most patients need a Pap smear every three years, but your provider may recommend a different schedule if you have certain risk factors, such as:

We offer Pap smears at all of our MetroWest clinics. Our provider will explain your results in clear language and help you understand the timing of your next recommended screening.

Sexually Transmitted Infections (STIs) Screening

Screening for sexually transmitted infections is an important way to protect your health. Testing helps identify sexually transmitted diseases early so they can be treated promptly, preventing complications and reducing the risk of unknowingly passing them on to your sexual partner.

During your visit, our provider will have a private, respectful conversation with you about your symptoms, risk factors, and any concerns you may have. We approach these discussions with sensitivity, so you can speak openly and get the care you need.

Based on your risks and symptoms, testing may include urine samples, swabs, or blood work, depending on the nature of your condition.

You may benefit from an STI screening if you are:

  • Starting a new sexual relationship
  • Having unprotected sex
  • Engaging in higher-risk sexual activity
  • Noticing symptoms such as vaginal discharge, burning, or pelvic pain
  • Seeking routine testing for peace of mind

If your test confirms an infection, we will guide you through your treatment options and help you understand what to expect. We make sure every question is answered, so you can feel confident in your sexual health and well-being.

Preparing for Your Gynecological Exam

A little preparation can help your visit feel comfortable. Before your appointment, take a moment to think about your personal and family medical history, as well as the details of your menstrual cycle.

Our provider may ask questions such as:

  • When was your last period?
  • How long do your periods usually last?
  • At what age did you start menstruating?
  • Are you currently sexually active?

There are no correct answers for any of these questions. We want all our patients to feel supported, safe, and at ease when talking about their health. These questions simply help us understand your body and provide the best care for you.
